What you should bring with you: 

• Time!
• Health insurance card (chip card)
• Seizure diary and all other important medical findings/documents
• Referral letter from a medical specialist such as a neurologist or pediatrician
• IMPORTANT: Referrals from general practitioners (Hausarzt) will not be accepted!
• The referral letter must include the following information:
  - Date of referral: The specialist must complete the letter PRIOR to the
    examination date.
  - Quarter: The quarter of the appointment
  - Stamp and signature of the specialist
  - Sample of referral letter (PDF) 

We ask for your patience when scheduling an appointment.  Due to high demand, waiting periods of several weeks might be unavoidable. 

 

We also  request that you please use the appropriate forms (e.g. AED prescriptions) in order to make the process more efficient.
